Insured Patients
We accept MEDICAID, MEDICARE, and most private insurance plans.
We do not accept the Free Care Card (yellow card) issued by LSU/UMC-NO, BCBS: Blue Advantage, Blue Connect, Signature Blue, and Humana: Gold Plus Diabetes & Heart, Total Care, and Humana – Ochsner.
If you have insurance, your insurance company will be billed and you will be responsible for the co-pay.

Uninsured Patients
Patients without insurance are eligible to receive services through our Sliding Scale System according to their household size and income.
You must provide proof of income at the time of your appointment.
Acceptable Forms of Proof of Income
- W-2 Income Tax Withholding Form
- Two current paystubs
- Unemployment Insurance Book or Letter
- Food Stamp Approval Letter
- Veterans Benefits Letter
